Purpose of Engagement

You understand the purpose of this engagement, including the specific learning objective(s) and the target audience for the service you have been asked to provide.

Engagement Legitimacy and Independence

You understand that this engagement is for a legitimate service based on a genuine need. Your engagement, including any associated payment, is not an incentive or reward for prescribing, administering, recommending, purchasing, or supplying any AstraZeneca product or service.

You are under no obligation to recommend, endorse, or prescribe AstraZeneca products as a result of this engagement. If your are receiving payment for this service, it reflects independent fair market value for the work you provide.

Disclosure to Attendees

You understand that this is an AstraZeneca-organised event and that your presentation is being delivered as a service to AstraZeneca. You agree to disclose your engagement with AstraZeneca, as well as any relevant relationships with other pharmaceutical companies, to attendees at the start of your presentation.

Meeting Details and Logistics

You are aware of the following logistical details:

• Time, date, location, and format (live or virtual)
• Agenda structure, including how questions will be handled
• Presentation requirements, including whether you need to prepare your own presentation and, if so, the requirement to submit this to AstraZeneca for approval at least 5 business days before the meeting. Payment Terms

You have agreed to the payment and payment terms that will be outlined in the Letter Agreement between you and AstraZeneca, including:

• Whether you wish to receive payment for this engagement
• Who payment will be made to (you or your employer)
• The capped hours for service delivery and any pre-defined preparation time, if applicable

Transparency reporting

If you are accepting payment for this engagement and/or other reportable transfers of value (such as travel, accommodation, or registration fees), you understand that these will be publicly disclosed in accordance with the mandatory Transparency Reporting requirements of Medicines Australia and/or Medicines New Zealand for a period of 3 years from the date of reporting.

Data Retention and Privacy

You understand that, to manage this engagement and meet transparency reporting obligations, your details will be retained in the AstraZeneca database, and you consent to this.

You also consent to AstraZeneca communicating with you electronically for the purposes of this engagement. This does not mean you will receive ongoing electronic communications from AstraZeneca unless you have separately consented to broader communications.

Conflicts of Interest and Employer Approval

You have disclosed to AstraZeneca any actual or potential conflicts of interest relating to you or your employer, and you agree to obtain any necessary approval from your employer to provide these services.

Letter Agreement

You understand that you will need to sign a Letter Agreement that describes the services you will provide to AstraZeneca, including the payment terms and conditions for this engagement.

You also understand that this agreement must be signed before you commence any work, including preparation.

Travel, Accommodation, and Expenses

You understand that any travel or accommodation required for this engagement will be organised and booked by AstraZeneca, and that AstraZeneca will only reimburse expenses that have been pre-defined and agreed in the Letter Agreement.

Recording

If your presentation or the meeting is being recorded:

You understand that a recording will be made and are aware of the purpose for which it will be used, including:

• The intended audience — whether the recording is for AstraZeneca internal use only or will be shared with external audiences (such as other healthcare professionals or patients)
• The intended purpose — how the recording will be used (e.g., educational or promotional activities)
• Retention period — how long the recording will be kept

You have provided verbal consent to the recording for the disclosed purpose. Written consent will also be obtained via the Letter Agreement.

Code of Conduct Compliance

You understand that this event and your service must be compliant with the Medicines Australia Code of Conduct and/or the Medicines New Zealand Code of Practice. This means:

• Approved information only — You must not include information about medicines or uses of medicines that are not approved by the TGA (Australia) and/or Medsafe (New Zealand). This applies to both your presentation content and any discussions during the event.
• Scientific and medical standards — All content must meet the principles of good medical and scientific practice. Statements must be accurate, balanced, fair, objective, unambiguous, and based on an up-to-date evaluation of all available evidence. Content must not be misleading, incapable of substantiation, or disparaging toward other companies or products.
• Mandatory requirements — Compliance with the applicable Code is mandatory. Failure to meet these requirements may affect AstraZeneca's ability to engage you for future services.

Approved Indications

You are familiar with the current approved indications for the medicine(s) and/or product(s) relevant to this event. If you need to verify approved indications, these can be found on the TGA website (Australia) and/or the Medsafe website (New Zealand).

Off-label Questions

If an audience member asks a question about an unapproved medicine or indication:

You understand that you are not permitted to answer or discuss this at an AstraZeneca-organised event. If the discussion moves toward off-label content, an AstraZeneca team member may step in to redirect the conversation.

A suggested response in this situation is:

"As this is an AstraZeneca event, I'm not able to discuss off-label use in this forum. I'd be happy to discuss this with you separately, outside of the AstraZeneca event. Alternatively, AstraZeneca's Medical Information team may be able to assist with your question."

Producing Materials

If you are creating or modifying a presentation or any other materials for this engagement:

You understand that your presentation and any supporting materials (such as handouts, worksheets, or supplementary resources) must be approved by AstraZeneca prior to the event, and that you must submit these at least 5 business days beforehand to allow sufficient time for the approval process.

You agree to include:

• A personal disclosure slide — outlining your relevant relationships with pharmaceutical companies
• The AstraZeneca disclaimer slide — as provided by AstraZeneca

If AstraZeneca requires modifications to your presentation or materials as part of the approval process, you agree to make these changes and resubmit for approval.

Patient Information and Copyright

You also confirm that any materials you prepare:

• Will not contain patient-identifiable information — no details that could identify an individual patient
• Will have appropriate copyright permissions — all third-party content (such as images, diagrams, or published data) has been used with the necessary authorisation

Adverse Event Reporting

You understand that AstraZeneca is obligated to report any adverse events, special situations, or product quality complaints relating to our products that arise during this engagement. This includes any such information contained within your presentation materials (such as case studies or images) as well as any that emerge during discussions.

If you become aware of any adverse events, special situations, or product quality complaints during the course of this engagement, you may either report this via the AstraZeneca Online Reporting Portal or notify the AstraZeneca team member present.

Publications (Medical Affairs only)

If this engagement involves the development of a publication:

You understand that the Letter Agreement will include additional terms relating to authorship, in line with the requirements of the International Committee of Medical Journal Editors (ICMJE). These terms will outline the criteria for authorship and your responsibilities in the publication process.

Confidentiality Declaration

If this engagement is an Advisory Board or involves the disclosure of confidential information:

You understand that you will be required to sign a Confidentiality Declaration in addition to the Letter Agreement. This must be signed before any confidential information is shared with you.

Off-Label Content at Scientific Exchange Meetings (Medical Affairs Only)

You have been advised if this engagement is a scientific exchange meeting organised by the AstraZeneca Medical Affairs team that includes discussion of off-label content. If you are unsure, you must seek clarifcation from your AstraZeneca point of contact for this event.

You understand that off-label content may only be discussed in limited, defined circumstances and that these circumstances have been communicated to you during the briefing. These requirements differ between Australia and New Zealand and you understand which applies to your service.

You agree that any off-label content included in your presentation or discussion will be:

• Scientifically balanced — presenting a fair and objective review of the available evidence
• Strictly non-promotional — not used for the purpose of promoting off-label use of any medicine

For the purposes of this requirement, promotion is defined as any statement, whether verbal, written, or displayed in a picture, which conveys the positive attributes of a product beyond a simple non-qualitative or non-quantitative description of the therapeutic category or approved indication, for the purpose of encouraging the usage of that product. This includes statements concerning efficacy, rate of adverse effects or other cautionary aspects, and comparative information.

In practice, this means your content should present evidence objectively and not advocate for or encourage the use of a medicine for an unapproved indication. If such content is perceived as encouraging use, AstraZeneca will request the content be removed.
